Prior to participating in athletics at CHS all athletes must complete, in full, the Athletic Participation Form. It includes:
1. Insurance – you may purchase student medical insurance or
2. Insurance Waiver – you need not purchase insurance if you
have coverage
3. Residency Statement – live in CHS area or have approved
attendance permit
4. Athletic Agreement – read and sign
5. Parents’ Consent – all athletes must have parents’ consent to
participate
6. Parents’ Medical Statement – medical history of athlete
7. Physical Exam – physical examination by a licensed doctor
8. Transportation Policies
